Case Management

Natalie has worked for the NHS and Social Care prior to moving into private practice in 2008.  Natalie is able to provide case management for the following clients:

• Cerebral Palsy

• Stroke (children/adults)

• Progressive neurological diseases

• Learning Disabilities (children/adults)

As a Case Manager, Natalie coordinates all aspects of the client's care, leisure, educational, therapeutic, and daily life needs.  Natalie has worked extensively with adults and children with complex needs and has an understanding of the impact this has on the client and all of the family.  She is experienced at sourcing and appointing qualified therapists, specialist nurses and personal assistants in the North Wales area.
